Documentation
Teams will need to submit the following information before the deadline of
September 15th:
- Completed application form
- Official Fall 2008 stamped and signed
team roster
- Approved travel papers (for out of state
teams)
- Tournament entry fee - cashier's check
or money order made payable to "FCSA"
Teams will need to have the following documents available at check-in:
- 5 Copies of Official Fall 2008 stamped
and signed roster
- Current notarized medical release form
for each player
- Current Fall 2008 player pass for each
player
- Guest player form for each guest player
(when applicable)
CHECK-IN WILL
BE FROM 6-8PM FRIDAY OCTOBER 10, 2008 AT THE FCSA OFFICE 1235 SOVEREIGN ROW,
SUITE C11. IF YOU ARE UNABLE TO CHECK IN THAT EVENING, WE WILL HAVE CHECK-IN
STARTING AT 7AM AT THE WESTSIDE FIELDS.
Venues
Host venues will be set in the near future. Age divisions will be assigned to
venues after the application deadline of September
15, 2008.
Accommodations
Host hotel will be announced. Hotel reservations may be made through
CSTT Travel Management at: CSTT
Reservations.
CSTT guarantees the lowest rates available.
